The PCR products are fragmented and labelled separately with two different reagents (Cy-3 and Cy-5 respectively) to allow the distinction between RHD and RHCE
An external control has been included to verify that the hybridisation has been performed correctly. This control will also help to place correctly the spot quantification grid. The chip also includes background hybridisation controls, which consist of spots containing solvent alone (50% DMSO) to allow the determination of background hybridisation.
Labelled PCR products will be applied to the microarray surface for hybridisation. Confocal scanner detects the bound DNA as fluorescence is emitted by the fluorophores upon excitation with a laser beam. 300 Background Ba ackground(med){a} 200 100 Signal 0 0 10000 20000 30000 Raw intensity(med){a}

Three new RHD genotypes found in Czech population in 2009 single point mutation in exon 1 /48G>C/ single point mutation in exon 5 /787G>A/ susp. hybrid gene RHD(1-8)-CE(9)-D(10) Písačka M, Reference Laboratory for Immunohematology Institute of Hematolgy and Blood Transfusion Prague, Czech Republic ISBT Berlin 2010
Case 1 Blood donor from ÚHKT Phenotyped as D positive /4+ reaction of 2 IgM anti-d/ C+c+E-e+Cw- BLOODchip v2.0 showed NO CALL in RHD due to the nucleotidic change 48 G>C in SNP BCV015 RHD gene sequencing confirmed this mutation and showed no other mutation in the rest of the gene Further the IVS5-38del4 change was detected Fragment analysis: hemizygous for RHD Extended D epitope typing was done with workshop MoAbs /20 IgM, 48 IgG/ - all reactions were strongly positive No antibodies available for epitopes 1.2, 5.5, 10.1-16.1 Positive i reaction with polyclonal l l anti-d Dfrom DIII Titre of IgM and IgG anti-d comparable with normal D /R 1 r/
Case 1 BCV015 nt 48 G>C Detail of sequence of exon 1 of RHD
Bloodchip v2.0 Case 1 No Call RHD due to a posible new SNPs combination i not previously described RHD exon 1 analysis by BLOODchip suggests a genotype Apparently non negative. RHCE exon 1 sequence: RHCE exon 1 sequence: 48G>C and homivs5 38del4.
Case 2 Pregnant woman from Brno Weak D in routine typing /2+ in tube test, 3+ in gel test/ C-c+E-e+Cw- PCR-SSP pattern as D Va type 2, Va, V type 7 and DCS D epitope typing with 6 MoAbs ID-Partial D Typing set: all positive, in Alba kit /12 MoAbs/: only LHM5717 negative D epitopes present: 1.1,1.2,3.1,6.3,6.5,8.1,9.1,15.1 RHD gene sequencing of exons 4 and 5 demonstrated a RHD gene sequencing of exons 4 and 5 demonstrated a single nucleotide polymorphism 787G>A in exon 5
Case 2 Detail of sequence of exon 5 of RHD
Case 3 Blood donor from ÚHKT Phenotyped as D positive /4+ reaction of 2 IgM anti-d/ C+c+E-e+Cw- BLOODchip v2.0 showed absence of signal for exon 9 BCVs Exon scanning showed absence of PCR product for exon 9 and presence for exon 10 Sequencing of exons 1-8 showed normal RHD sequence Fragment analysis: hemizygous for RHD Extended D epitope typing was done with workshop MoAbs /20 IgM, 48 IgG/ - all reactions were strongly positive No antibodies available for epitopes 1.2, 5.5, 10.1-16.1 Positive i reaction with polyclonal l l anti-d Dfrom DIII Titre of IgM and IgG anti-d comparable with normal D /R 1 r/
Bloodchip v2.0 Case 3 Bloodchip showed a genotype exon 9 variant, a phenotype D variant. RHD exon 9 analysis by BLOODchip suggests a D(1 8) CE(9) D(10) conversion.
Conclusions (1) Three genotypes described here were not found in Rhesus Base and not reported in the literature Case 2 serology correspond to weak D Cases 1 and 3 have normal D qualitative and quantitative serology results Amino acids exchanges in Case 1 and 2 correspond to RhCcEe protein cdna sequencing of Case 3 should be done to clarify exon cdna sequencing of Case 3 should be done to clarify exon 9 sequence
Conclusions (2) Case 1 could be hybrid gene RHCE(1)-RHD(2-10) Case 2 could be either point mutation or hybrid gene with smaller RHCE exchange in exon 5 Case 3 could be hybrid gene RHD(1-8)-CE(9)-D(10) D(10) or RHD(1-7)-CE(8-9)-D(10) hypothesis RhC specific aa Cysteine in position 16 does not influence RhD expresion RHD specific sequence in exon 9 is not necessary for RHD specific sequence in exon 9 is not necessary for normal RhD expression
